We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Having read the Patient Information and Summary Care Record information please register by completing the New Patient Questionnaire, NHS GMS 1 form and Patient Online Access (optional). You will need to sign these documents and present them either at our Colne or Earby Surgery along with identification.
Please ensure that you have a month’s supply of your medication from your last practice before registering with us.
How We Work
All of our clinicians work as a team. Your registration will be with The Pendle Medical Partnership, rather than with a specific doctor. However, many patients prefer to get to know one doctor, and our system allows for that too. You can choose to see another doctor in the practice e.g. a male/female doctor, or a doctor with specific expertise. However please be aware that you may have to wait a little longer to talk to the GP of your choice.
